IMPORTANT UPDATE: On September 15th 2025, the practice will be moving to Providence St. Joseph Medical Center at 501 S. Buena Vista Street in Burbank. The ENT clinic will be located in 1 North in the Neurosciences Department. The phone number there is 818-847-6049. All phone calls, including appointment requests, prescription refills, surgery scheduling and all other concerns should be directed to the new office number as of 9/15/25.

Of note, the new office will be unable to accept payments for dates of service prior to September 15th 2025 by phone. Options for making payments include the following: 1) Returning a check or credit card information with the invoice received in the mail, or 2) Accessing the Patient Portal to make a payment online. To do this, click on “Patient Portal” above, then “Existing Patient Registration” and complete the mandatory fields. This process requires an email on file with our office. If you're unable to complete Patient Portal registration, it is likely because we do not have an up-to-date email address in your chart. Please call our new office number or send us an email or message through the website to provide this information and we will activate the Portal for you. An email link to complete the setup will be sent and you may follow the prompts to make a payment online.
